Job Summary

We are seeking a highly skilled Clinical Social Worker to join our team at Olmsted Medical Center-MAIN. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.

Key Responsibilities
  • Assessment and Treatment

Conduct thorough assessments of patients' mental health needs, develop and implement effective treatment plans, and provide individual and group therapy sessions.

Case Management

Work closely with patients, families, and healthcare providers to coordinate care, ensure seamless transitions, and promote optimal health outcomes.

Community Resources

Identify and connect patients with community resources, support groups, and other services to enhance their overall well-being.

Quality Improvement

Participate in quality improvement initiatives, contribute to the development of policies and procedures, and engage in ongoing professional development to stay current with best practices.

Collaboration and Communication

Work collaboratively with interdisciplinary teams, communicate effectively with patients, families, and healthcare providers, and maintain accurate and timely documentation.

Requirements
  • Education and Licensure

Master's degree in Social Work (MSW) from an accredited program and current licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Minnesota.

Experience and Skills

1-2 years of experience in a healthcare setting, with a strong background in mental health assessment, treatment, and case management. Excellent communication, interpersonal, and problem-solving skills, with the ability to work effectively in a fast-paced environment.

Certifications and Training

BLS certification within 90 days of hire and completion of any required training programs.
