Job Title: Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Bancroft. As a Physical Therapist, you will provide direct, consultative, and supportive diagnostic and therapeutic physical therapy services to individuals with neurological impairments.

Responsibilities:
  • Provide direct and consultative physical therapy services, including assessments and treatments in accordance with regulations, standards, and guidelines defined by the American Physical Therapy Association (APTA) Code of Ethical Practice, N.J. Physical Therapy Practice Act, Division of Developmental Disabilities (DDD), the Individual Program Plan (IPP), Health Insurance Portability & Accountability Act (HIPAA), and established Bancroft policies and procedure.
  • Conduct diagnostic assessments of need for physical therapy services and develop treatment plans for individual persons served, including determining the need for therapeutic intervention, reviewing individual records, administering standardized testing, performing formal and informal testing, conducting specialized screenings, interpreting assessment results, and providing recommendations for service delivery.
  • Develop and modify treatment plans based on individual abilities and needs, and provide reassessment of persons served, focusing on targeted areas of need at established intervals.
  • Plan and develop goals specific to physical therapy and treatment services that are achievable, functional, measurable, and will be effectively utilized in each individual's rehabilitation program.
  • Provide direct and consultative treatment services in physical therapy to persons served, including effectively utilizing available information regarding therapy and treatment for each person served, providing assistance in the development of physical therapy and treatment-specific goals and objectives, and assisting program management in the design of functional strategies and the need for adaptive equipment that enhances learning and creates an environment best suited to individual profile and needs.
  • Provide individual, group, and integrated direct treatment, therapy, and/or special rehabilitation skills to persons served, prompt and/or assist each person in the achievement of goals while actively encouraging the independence and competency of each person within his/her ability limit, plan and schedule frequency and duration of sessions in accordance with individual needs, and provide suggestions regarding the utilization of techniques and/or materials to support and enhance functional abilities.
  • Recommend appropriate adaptive equipment to facilitate optimal functioning, provide recommendations regarding positioning, identify barriers that may impede individual progress, and recommend environmental accommodations to support functioning and facilitate the carryover of therapeutic goals.
  • Demonstrate and instruct persons served and staff in the proper use of functional and adaptive equipment, devices, and physical therapy techniques, and utilize a system of documentation that is professional, efficient, accountable, and conforms to federal, state, and other applicable policies and procedures.
  • Provide training on a variety of topics and specific to physical therapy to persons served and staff members regarding the implementation and monitoring of treatment plans, and facilitate a team approach to program development, implementation, and monitoring by effectively collaborating with all specialty/clinical disciplines, parents/guardians, residential and day program staff, etc., to identify and best meet the individual rehabilitation needs of each person served.
  • Actively participate in monthly team/departmental meetings, etc., and serve as a member of the Trans-Disciplinary Team (TDT) process in conjunction with the Individual Program Plan (IPP).
  • Communicate results of assessments and treatment plans to persons served, family members, residential and day program staff, external case managers, etc., and provide crisis intervention, as needed, through de-escalation techniques, staff support, and assistance.
  • Ensure the site operates in accordance with the organization's quality standards for safety, security, and cleanliness, as well as applicable state licensing and accreditation standards.
Qualifications:
  • Master's degree in Physical Therapy and a current N.J. State Physical Therapist license required.
  • Minimum of one year of work experience in physical therapy preferred, working with neurologically-impaired individuals.
