About the Role:

As an MDS Coordinator, you will be responsible for assessing residents' needs and creating personalized care plans. You will work closely with our interdisciplinary team to ensure seamless care delivery and promote a culture of compassion and respect. Key Responsibilities:

• Manage the Resident Assessment Instrument (RAI) process to assess residents' individual needs and create unique care plans
• Promote cross-functional communication to ensure care plans are followed and residents receive high-quality care
• Ensure accurate reimbursement for resident care services and stay up-to-date with federal, state, and local standards, guidelines, and regulations
