Job Title: Nurse Transition Leader

Job Summary:

Otterbein SeniorLife is seeking a skilled Nurse Transition Leader to provide leadership and supervision to licensed healthcare areas and staff at assigned locations. The ideal candidate will have a strong background in nursing and experience in long-term care and post-acute environments.

Responsibilities:

  • Obtain daily reports from nurses and conduct effective daily rounds
  • Discuss observations and concerns about clinical compliance with nurses or care coordinators
  • Monitor compliance of collection and submission of MDS data
  • Review and ensure staff compliance with in-service requirements and self-development programs
  • Monitor quality measures and coordinate quality improvement initiatives
  • Monitor clinical budgets and act as a fiscal steward
  • Complete incident reporting and follow-up
  • Act as a liaison with medical directors and complete monthly reports and quarterly QA initiatives
  • Effectively supervise staff in all areas of employment in partnership with Human Resources
  • Act as a liaison with healthcare vendors
  • Work on the floor when needed
  • Act as a liaison with families when appropriate

Qualifications:

  • Education: Bachelor of Science in Nursing (BSN)
  • Licensure/Certification: Current Ohio RN license; RN license in Indiana a plus
  • Experience: Minimum 3 years of Director of Nursing experience in long-term care/nursing home environment required; experience as an MDS nurse a significant plus; experience in post-acute environments also a plus

Special Skills and Requirements:

  • Proven track record of strong Ohio Department of Health survey results
  • Track record of recruiting and developing high-quality clinical talent
  • Ability to be flexible and adapt quickly to change
  • Experience with Electronic Medical Records; PCC experience highly preferred
  • Must be detailed-oriented and have excellent literacy and writing, documentation, communications, and interpersonal abilities
  • Must be able to cope with mental and emotional stress related to working with persons who are ill, disabled, elderly, and/or emotionally upset
  • Must be able to communicate in English
