Optimal Care Coordination Opportunity
UnitedHealth Group, a leading health care company, is seeking a skilled Registered Nurse to join our Senior Community Care (SCC) team. As a Field Clinician RN, you will play a vital role in providing high-quality care to our most vulnerable patients in a facility setting.
Key Responsibilities:
- Ensure accurate and complete documentation of patient care
- Collaborate with the interdisciplinary care team to develop and implement comprehensive care plans
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
- Assist with benefit determination and coordination of care services
- Participate in facility-based meetings and contribute to quality improvement initiatives
- Develop and conduct in-services for selected audiences to enhance knowledge and skills
- Assist with medication reconciliation, nursing rounds, and patient documentation
- May write verbal orders from Providers in compliance with state RN practice laws and individual nursing facility practices
Requirements:
- Associate degree or higher in Nursing (RN)
- Active and unrestricted license in the state where you reside, or ability to obtain by start date
- Certified in Basic Life Support
- Ability to complete physical requirements, including lifting a 30-pound bag and performing physical assessments
- Access to reliable transportation
- Ability to travel between facilities
Preferred Qualifications:
- 1+ years of clinical experience in an acute care setting
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
- Saline Lock insertion experience
- Understanding of Geriatrics and Chronic Illness

Compensation and Benefits:
The hourly range for this role is $28.03 to $54.95 per hour. UnitedHealth Group offers a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and k contribution (all benefits are subject to eligibility requirements).
