We are seeking a dedicated and accomplished RN Hospice Specialty Registered Nurse to join our team at Masonicare Home Health & Hospice in the Branford, CT area.

Job Summary

The RN Hospice Nurse will ensure quality and safe delivery of hospice nursing services to patients in their homes or assisted living settings. This role involves developing, implementing, and evaluating patient care plans, making necessary revisions to address any problems, and coordinating care with other healthcare professionals.

Responsibilities
  • Develop and implement individualized patient care plans that meet the unique needs of each patient
  • Provide education, supervision, and counseling to patients and their families on home nursing procedures and other care needs as appropriate
  • Assure patient and family involvement in the development of the care plan whenever possible
  • Supervise client care rendered by Licensed Practical Nurses and Home Health Aides in the provision of delegated duties and conduct supervisory visits per regulatory guidelines
  • Ensure timely and accurate computer documentation of patient information to facilitate the development of the OASIS data set, orders for service, creation of time sheets, and other required patient documents and reports
Requirements

To be considered for this role, applicants must have experience in hospice and/or home care, and hold a current RN license. New graduates will not be considered.

Benefits

Masonicare offers a competitive annual salary and eligibility for weekly productivity bonuses. Our nurses demonstrate timely and accurate computer documentation of patient/job information to facilitate the development of the OASIS data set, orders for service, creation of time sheets and other required patient/employment documents and reports.

Estimated Salary Range

$75,000 - $90,000 per year based on experience.
