We are seeking a compassionate and skilled End-of-Life Care Specialist to join our team at Gentiva Hospice. In this role, you will have the opportunity to make a meaningful difference in the lives of patients and their families.
Job SummaryThe End-of-Life Care Specialist will provide high-quality care to patients in their homes, hospitals, or long-term care facilities. This includes assisting with daily living activities, providing emotional support, and helping patients maintain their independence.
Responsibilities- Assist patients with personal care activities such as bathing, dressing, and grooming
- Provide emotional support and companionship to patients and their families
- Help patients maintain their independence by assisting with daily living activities
- Collaborate with healthcare teams to develop and implement patient care plans
- Document patient progress and provide updates to healthcare teams
To be successful in this role, you must have:
- A current Certified Nursing Assistant (CNA) license
- Six months of experience in patient care, preferably in hospice, home health, or geriatric settings
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassion, empathy, and a commitment to delivering high-quality care
