Key Responsibilities
  • Patient Assessment and Treatment Planning: Conduct comprehensive psychosocial assessments to identify patient needs and develop individualized treatment plans in collaboration with our multidisciplinary team.
  • Clinical Services and Intervention: Provide evidence-based interventions to address patient psychosocial needs, including counseling, education, and support.
  • Care Coordination: Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Practice Management: Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and services.
Requirements
  • Education: Master's Degree in Social Work (MSW) from an accredited program.
  • Experience: 1-2 years of experience in a healthcare setting, preferably in a social work or counseling role.
  • Licensure: Licensed Master's Social Worker (LMSW) in Kansas, with the ability to obtain licensure in Missouri within 90 days.
