Job Summary

We are seeking a highly skilled and compassionate Care Coordinator to join our team at HomeWell Care Services of Indianapolis. As a Care Coordinator, you will play a vital role in managing client care plans, coordinating services, and ensuring that clients receive the highest level of care.

Key Responsibilities
  • Develop and manage individualized care plans to meet client needs and preferences.
  • Coordinate and schedule services between clients, families, caregivers, and healthcare providers to deliver comprehensive care.
  • Conduct initial and ongoing assessments to determine client needs and preferences.
  • Oversee caregiver assignments to ensure the right caregiver is matched to each client.
  • Monitor and evaluate the quality of care provided, adjusting care plans as needed.
  • Provide in-home visits to supervise care, evaluate service delivery, and maintain high standards of care.
  • Serve as the primary point of contact for clients and families, addressing any concerns and ensuring client satisfaction.
Fieldwork and Administrative Support
  • Visit clients' homes to supervise caregivers and assess client satisfaction.
  • Monitor in-home care to ensure caregivers follow care plans and deliver quality service.
  • Assist with recruiting, hiring, and onboarding of caregivers to ensure a consistent pool of qualified personnel.
  • Maintain accurate and up-to-date client records and documentation.
  • Support the management team in developing and implementing care policies and procedures.
Requirements
  • Minimum of two years of experience in home care, home health, or a related field.
  • Reliable transportation, current auto insurance, and a valid driver's license are required.
  • Exceptional written and verbal communication skills, with the ability to effectively engage with clients, families, care providers, and professionals from diverse backgrounds.
  • Strong attention to detail and follow-through, with the ability to take initiative, solve problems, and exercise sound judgment.
  • Proven skills in supervising, assessing, and evaluating care providers, as well as advocating on behalf of clients and their families.
  • Passionate about helping others, with a proactive, driven personality.
  • Must maintain a positive and professional appearance and demeanor at all times.
  • Successful completion of a background check and tuberculosis test is required.
  • Must comply with drug screening requirements.
