About the Role:
We are seeking a highly skilled Registered Nurse to join our team as a Home Health Nurse II. As a member of our interdisciplinary team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Job Summary:
This position requires a registered nurse with a strong foundation in home health care principles and practices. The ideal candidate will possess excellent assessment and critical thinking skills, be able to prioritize tasks effectively, and demonstrate a commitment to delivering high-quality patient care.

Key Responsibilities:
- Assess patient needs, develop individualized plans of care, and implement evidence-based interventions to promote optimal patient outcomes.
- Collaborate with multidisciplinary teams to coordinate care, share information, and ensure seamless transitions between settings.
- Educate patients and their families on disease management, medication adherence, and self-care techniques to empower them to take an active role in their health.
- Participate in quality improvement initiatives, contribute to the development of policies and procedures, and engage in ongoing professional development to stay current with best practices in home health care.

Requirements:
- A valid California RN license and current BLS certification are required. Prior experience in home health care or a related field is preferred.
- Strong knowledge of nursing practices, regulations, and standards of care in home health settings is essential.
- Excellent written and verbal communication skills, with the ability to communicate effectively with patients, families, and healthcare professionals.
- Ability to maintain accurate records, adhere to documentation policies, and participate in quality improvement initiatives.
