Home Health Nurse Specialist

We are seeking an experienced Home Health Registered Nurse for a 13-week travel assignment in Santa Clara, CA. As a Home Health Nurse Specialist, you will be responsible for providing high-quality patient care in the comfort of their own homes. You will work independently with patients to develop and implement personalized care plans tailored to their specific needs.
Responsibilities:
- Develop and implement personalized care plans for patients
- Provide high-quality patient care in the comfort of their own homes
- Work independently with patients to assess their needs and develop care plans
- Collaborate with other healthcare professionals to ensure comprehensive care
Requirements:
- BSN or Associate's degree in Nursing
- Active RN License for this state
- At least 1 year of experience in home health care required
- BLS Certification and ACLS Certification
- Experience with Homecare Homebase system required
- IV start experience highly desirable
- Oasis experience required
