About the Role

We are seeking a highly skilled Care Transition Liaison to join our team at LHC Group. As a Care Transition Liaison, you will play a critical role in facilitating the transition of patients from hospital or physician's office to our home health or hospice services.

Key Responsibilities
  • Facilitate Referrals: Collaborate with physicians and case managers to identify patient needs and obtain approval for admission to our agency.
  • Coordinate Care Transitions: Work with hospitals, physicians, and our agency to ensure seamless transitions of care.
  • Communicate with Patients and Families: Educate patients and families on our services and ensure they understand their care options.
  • Document Activities and Productivity: Accurately document all activities and productivity within our available systems.
  • Other Duties: Perform other duties as assigned by your manager.
Requirements
  • Experience: Prior experience in hospital, physician's office, home health, or hospice settings is preferred.
  • Licensure: LPN/LVN or licensed medical professional is preferred.
  • Transportation: Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation are required.
