Job Title: MDS Assessment Coordinator RN

Keota Health Care is seeking a skilled MDS Assessment Coordinator RN to join our team at our Skilled Nursing Facility in Keota, IA.

Job Summary:

The primary purpose of this role is to conduct and coordinate the development and completion of resident assessments in accordance with state requirements and facility policies.

Responsibilities:
  • Routinely assess residents according to pre-set schedules or as needed for condition changes, hospital returns, etc.
  • Responsible for accurate observation, assessment, and communication of condition changes to appropriate personnel
  • Assure completion of assessments and CAA's by other departments such as Social Services, Activities, and Dietary, and obtain corresponding signatures
  • Complete other sections of MDS, quarterlies, CAA's and care plans, etc., in the absence of other personnel
  • Initiate care plans based on resident needs identified in the Resident Assessment Protocols and update care plans according to guidelines
  • Prepare and distribute MDS schedule to interdisciplinary team
  • Transmit MDS to the state weekly (or as required) and maintain accurate records, including a transmittal log
  • Plan, organize, develop, and direct the overall operation of the Nursing Service Department
  • Participate in interviewing and screening residents for admission to the facility
  • Review nurses' notes to ensure proper documentation is maintained relating to resident's treatment, medications, and conditions
  • Make daily rounds for observation of the care of residents
  • Talk with physicians and visiting selected residents
  • Assist the Resident Assessment/Care Plan Coordinator in planning, scheduling, and revising the MDS, including the implementation of RAP's and triggers
Qualifications:
  • Registered Nurse RN of this state
  • Over a year's experience in Long-Term Care as an MDS assessment coordinator
