Keota Health Care is seeking an MDS Assessment Coordinator for our Skilled Nursing Facility in Keota, IA

The primary purpose of the job position is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of the state and the policies and goals of this facility.

This is a full-time position, part time as MDS and 1 day per week as a nurse on the floor

Sign On Bonus: Earn up to $10k based on position/shift accepted

Buddy Bonus: $1000 for every FT or PT nursing staff member you bring with you

Responsibilities

Routinely assess residents according to pre-set schedules, or on an “as needed” basis for condition changes, hospital return, etc.

Responsible for accurate observation, assessment, and communication of condition changes to appropriate personnel

Assures completion of assessments and CAA's by other departments such as Social Services, Activities, and Dietary, and obtains corresponding signatures

Complete other sections of MDS, quarterlies, CAA’s and care plans, etc., in the absence of other personnel

Initiate care plans based on resident needs identified in the Resident Assessment Protocols and update the care plans according to guidelines

Prepares and distributes MDS schedule to interdisciplinary team

Transmit MDS to the state weekly (or as required) and maintain accurate records, including a transmittal log

Plan, organized, develop, and direct the overall operation of the Nursing Service Department

Participate in interviewing and screening residents for admission to the facility

Review nurses’ notes to ensure proper documentation is maintained relating to resident’s treatment, medications, and conditions

Make daily rounds for observation of the care of residents

Talking with physicians and visiting selected residents

Assist the Resident Assessment/Care Plan Coordinator in planning, scheduling, and revising the MDS, including the implementation of RAP’s and triggers

Qualifications:

Registered Nurse RN of this state

Over a year's experience in Long-Term Care as an MDS assessment coordinator.

Pay: $40.00 per hour. Negotiable
