2. Clinical Pharmacy Practice: Conduct thorough medication order reviews and patient assessments to identify and resolve medication-related issues. Collaborate with healthcare professionals to ensure optimal medication therapy management and patient safety.

3. Patient-Centered Care: Engage in direct patient care initiatives, ensuring timely and appropriate medication therapy tailored to individual patient needs. Work collaboratively with healthcare teams to enhance patient outcomes.

4. Inter-professional Collaboration: Foster effective communication among healthcare team members to ensure seamless care transitions. Participate in departmental meetings and support initiatives aimed at improving pharmacy practices.

QUALIFICATIONS:

Required Skills:

Education:

Bachelor of Pharmacy, Doctor of Pharmacy, or equivalent advanced degree required.

Experience:
