Behavioral Health Therapist Bilingual Spanish

At Wonderful Center for Health Innovation, we are seeking a highly skilled Behavioral Health Therapist to join our team. As a key member of our primary care team, you will play a vital role in helping patients improve their physical and mental health.
The ideal candidate will have a current/valid LCSW or LMFT license issued by the State of California Board of Behavioral Sciences and be fluent in Spanish. Experience working with adults, teens, and pediatric patients is preferred. Knowledge of social service agencies, state regulations, and APA standards is also essential.
Responsibilities will include:
- Supporting and closely coordinating mental health care with patients' primary care providers and other treating mental health providers.
- Screening and assessing patients for common mental health, substance abuse disorders, and unmet social needs.
- Providing patient education about common mental health and substance abuse disorders and available treatment options.
- Monitoring patients for changes in clinical symptoms and treatment side effects or complications.
- Developing relationships with community providers and facilitating referrals for clinically indicated services outside the primary care clinic.
- Supporting psychotropic medication management prescribed by PCPs, focusing on treatment adherence, side effects, and other complications, and effectiveness of treatment.
- Providing brief interventions using evidence-based techniques such as Behavioral Activation, Problem-Solving Treatment, Motivational Interviewing, or other treatments appropriate for primary care settings.
- Providing or facilitating in-clinic or outside referrals to evidence-based psychosocial treatments (e.g., CBT, IPT) as clinically indicated.
- Participating in regularly scheduled caseload consultation with the psychiatric consultant and communicating resulting treatment recommendations to the patient's PCP.
- Facilitating patient engagement and follow-up in care.
- Tracking patient follow-up and clinical outcomes using a registry.
