We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager in Home Health. As a key member of our care team, you will be responsible for providing high-quality patient care and collaborating with our interdisciplinary team to develop and implement individualized care plans.
Responsibilities- Perform patient assessments and develop comprehensive care plans in collaboration with the care team.
- Make referrals to other disciplines as indicated by the patient's identified needs or document rationale for not doing so.
- Promote patient health and independence through teaching and rehabilitative measures, assisting patients in learning self-care techniques.
- Supervise LPNs and HHAs, ensuring high-quality care delivery.
- Complete timely and accurate documentation, regularly communicating patient progress to the clinical manager and care team.
- Plan and provide staff education, participating in clinical development and continuing education programs.
- Perform on-call responsibilities and provide on-call services to patients/families as assigned.
- One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.
- Current RN license, specific to the state(s) you are assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
