We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Clinical Care Coordinator for our Hospice service line. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care and ensuring the best possible outcomes for our patients and their families.
Key Responsibilities:
- Provide comprehensive patient assessments and develop individualized care plans to meet the unique needs of each patient.
- Collaborate with the healthcare team to coordinate care and services, ensuring seamless transitions and optimal patient outcomes.
- Conduct regular patient visits and assessments to monitor progress and adjust care plans as needed.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care.
- Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving outcomes.
Requirements:
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
- Current State Licensee as a Registered Nurse.
- 1 year of experience in a clinical care setting, with a preference for hospice or home health experience.
- Nursing skills as defined by accepted nursing standards.
- Ability to handle multiple priorities, documentation requirements, and deadlines.
- Reliable transportation to perform job tasks.
