As a Hospice RN/ RN Case Manager, your responsibilities will include:

  • Reporting directly to the Administrator, Executive Director, or Patient Care Manager Senior, while maintaining primary oversight and professional management of each patient.
  • Serving as the main point of contact between the physician, patient/family, and hospice team.
  • Delivering direct patient care services to hospice patients and providing supportive care to their families with a manageable caseload, fostering a trusting and compassionate relationship.
  • Empowering caregivers by educating them on the care of their loved ones.
  • Collaborating with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to ensure comfort, peace, and dignity.
  • Assessing patient and family needs and assisting in the development and implementation of individualized care plans.
  • Maintaining accurate clinical records for all assigned patients.
  • Participating in quality assessment and performance improvement initiatives within the company.
About You

Hospice Registered Nurse Qualifications:

  • A compassionate heart: empathetic, kind, and understanding towards patients and their families.
  • Comfortable providing care in various settings as designated by the patient.
  • Ability to demonstrate flexibility and composure while supporting others during challenging times.
  • RN license in the state of employment (or eligibility to obtain).
  • A minimum of 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology is strongly preferred.
  • Additional preferred experience that may enhance your success in a hospice role includes: hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing.
  • A valid driver's license and automobile insurance.
  • Current CPR certification.
