Position Overview

Director of Social Work (Full Time)

Greenbrook Manor Care and Rehabilitation LLC is in search of a dedicated Director of Social Work. This pivotal role involves delivering medically-related social services aimed at achieving and maintaining the highest possible physical, mental, and psycho-social well-being of our residents.

Key Responsibilities:
  • Design and execute comprehensive case management services for all residents.
  • Engage with families (with resident's consent) to communicate health updates, current objectives, discharge planning, and to encourage involvement in care planning.
  • Maintain detailed progress notes in accordance with federal and state regulations for Long Term Care.
  • Promote the development of relationships between residents and staff by educating staff on understanding and addressing the unique needs of each resident.
Qualifications:
  • Must possess an LCSW or CSW designation. A valid social work license is required.
  • A Bachelor's degree from an accredited Social Work program is necessary.
  • Experience in subacute and geriatric care is preferred.
  • A minimum of one year of experience working with seniors in a supervisory role within a hospital, skilled nursing facility, or similar medical environment.
