Job Summary

Mass General Brigham Healthcare at Home is seeking a skilled Charge Nurse to join our team. As a Charge Nurse, you will be responsible for providing high-quality, clinical services to patients in various settings within the community. Your primary focus will be on delivering patient care that adheres to our organizational philosophy, policy, and standards of nursing and community health practice.

Key Responsibilities
  • Develop a plan of care that meets the patient's needs, safety, and diagnosis for optimal patient outcomes.
  • Provide teaching and education to patients and their families, evaluating their learning style and adapting your approach accordingly.
  • Initiate discharge planning, involving the patient, family, and multidisciplinary clinical care team, and accessing community resources as needed.
  • Establish patient schedules in collaboration with others, adjusting and communicating them timely and in collaboration with the patient's home-based clinical care team.
  • Communicate patient information and changes in status/plan of care to the care team, including the APP &/or MD as needed, and document them timely.
  • Reconcile medications accurately and timely at admission, discharge, and transfer, and integrate any changes into the patient's care plan and document them in the electronic health record.
  • Make referrals to other disciplines and community resources to maximize patient safety and outcomes.
  • Identify patients for multi-disciplinary case conference/rounding as needed.
  • Prepare and communicate patient information to appropriate care team members and others as needed via effective communication methods.
  • Attend staff meetings, competencies, mandatory compliance training, and educational meetings regularly, supporting clinical initiatives and special training.
  • Update software by maintaining the latest version of software applications as defined by the IS department.
  • Adhere to Point of Care standards for submission of data, no later than midnight of the day of the visit/admission, and sooner if patient condition warrants.
  • Accurately document initial assessments and visit notes, ensuring high-quality patient care in accordance with program standards and regulatory compliance as applicable.
  • Order medical supplies and equipment according to allowable guidelines.
  • Rotate Charge Nurse responsibilities.

Qualifications

Education/Degree Requirements:
* Graduate of an approved School of Nursing

Licensure, Certifications or Registration:
Required: Current Massachusetts license as Registered Nurse

Experience:
* Minimum of one-year recent nursing experience in acute or emergency setting required.
* Home Care experience a plus.
* Travel required within designated geographic area.
* Ability to work with various computer software applications and technologies required.
* Strong interpersonal and customer service and ability to work with a diverse population.
* Ability to communicate effectively in writing, verbally, and electronically.
* Ability to work as a member of an interdisciplinary team of healthcare providers.
* Ability to organize and prioritize work.
* Ability to work independently, be self-directed, and adapt to unpredictable circumstances.

Physical Environment
Primarily will work in patients' homes. Potential exposure to blood, blood products, body fluids, and chemical hazards. Attend meetings in a variety of external organizations, healthcare facilities, hotels, and other venues.
