Licensed Clinical Social Worker

We are seeking a highly skilled Licensed Clinical Social Worker to join our team at Rehab Without Walls Neuro Rehabilitation. As a key member of our interdisciplinary team, you will provide counseling services to support individuals with physical, mental, cognitive, and emotional adjustment through recovery.
Responsibilities- Perform comprehensive evaluations to assess social, emotional, and support needs of patients and their families.
- Develop measurable objectives in conjunction with the rehab team.
- Assist physicians and other health team members in understanding significant social and emotional factors related to patient health problems.
- Instruct and counsel patients and families in treating and coping with social and emotional responses connected with illnesses.
- Participate in care coordination activities and act as a resource to other health team members in identifying and resolving patient needs.
- Identify and assist patients and families in utilizing appropriate community resources to achieve measurable objectives.
- Master's Degree in Clinical Social Work (LCSW)
- Current Licensed Clinical Social Worker Licensure in the state of practice
- Minimum of one year experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or acute diagnosis
- CPR certification required
- Ability to lift 50 pounds
- Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
