About the Role:
We are seeking a skilled RN Case Manager to join our team at Bristol Hospice. As a RN Case Manager, you will work closely with patients, families, and healthcare teams to deliver high-quality patient care and support. This role requires strong communication and problem-solving skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities:
* Assess patient needs and develop individualized care plans
* Provide direct patient care, including medication administration and symptom management
* Collaborate with interdisciplinary teams to ensure comprehensive patient care

Qualifications:
* Current RN license in good standing
* 1 year of experience in hospice or home health preferred
* Strong observation, verbal, and written communication skills
* Ability to work in a fast-paced environment with multiple priorities
