As a Physical Therapist - Outpatient Care Specialist, you will be responsible for evaluating patients, developing treatment plans, and providing physical therapy services to help patients achieve optimal function and mobility.

Responsibilities:

* Evaluate patients based on departmental standards
* Conduct and modify evaluations based on patient symptoms, activity level, and medical status
* Treat patients with appropriate therapeutic exercises and modalities
* Administer physical agents and manual therapeutic techniques to relieve pain and attain established goals
* Educate patients and families on home exercise programs and functional activities
* Determine when patients have achieved maximal benefit from treatment and recommend discharge
* Document patient information in accordance with insurance guidelines and department policies

Requirements:

* Graduate of an accredited Physical Therapy program with a minimum of a B.S.
* Current Connecticut license in Physical Therapy or Occupational Therapy
* Certified Manual Lymph Drainage Therapist required
* Strong English communication skills, both verbal and written
* Personal computer skills and proficiency in Windows-based programs
* Reliable transportation to travel between multiple work sites
