Job Title: Community Liaison Worker Level II-Outpatient Behavioral Health Child and Adolescent

At NYC Health + Hospitals, we are committed to delivering high-quality care to our patients. As a Community Liaison Worker Level II-Outpatient Behavioral Health Child and Adolescent, you will play a critical role in ensuring that our patients receive the best possible care.

About the Role:

This position is responsible for providing community liaison services to patients and families in the outpatient behavioral health setting. The successful candidate will work closely with the interdisciplinary team to ensure that patients receive the necessary support and resources to achieve optimal health outcomes.

Key Responsibilities:
  • Perform patient outreach to address patient needs and ensure follow-up services are provided
  • Call patients after discharge to verify safe transitions and ensure patient follow-up appointments with PCP (Lincoln and outside providers) as appropriate
  • Support patients in need of assistance with one-on-one contact
  • Ensure that patients with high-risk diagnoses receive the appropriate care based on best practices
  • Help patients navigate through transitions in the healthcare system
  • Work within the organization and through external services to eliminate barriers to healthcare
  • Collect data and submit reports on activities/interventions and outcomes as identified
Requirements:

To be successful in this role, you will need:

  • One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining community services
  • Completion of an approved six-month training program of combined classroom training and on-the-job experience in community liaison work, plus six months of full-time experience as described above
