Job Summary:

We are seeking a compassionate and organized Social Support Coordinator to join our team. The successful candidate will be responsible for assessing clients' social support needs, developing and implementing strategies to address these needs, and connecting clients with relevant resources.

Main Responsibilities:

  • Needs Assessment: Conduct thorough assessments of clients' social support needs, including housing, employment, and education requirements.
  • Support Planning: Develop and implement personalized support plans that address clients' specific needs and goals.
  • Resource Connection: Facilitate connections between clients and relevant social support services, including housing programs, employment services, and educational institutions.
  • Outcome Evaluation: Track and analyze clients' progress, identifying areas for improvement and adjusting support plans as needed.

Requirements:

  • Education: Bachelor's degree in a social science or related field from an accredited college or university.
  • Experience: 2 years of work experience in social support coordination or a related field.
  • Skills: Excellent communication and interpersonal skills, ability to work effectively in a fast-paced environment, and proficiency in data management systems.
