Job Summary:

The LPN/LVN will work under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. This includes assessing patient needs, developing and implementing care plans, and providing education to patients and their families.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Assess patient needs and develop individualized care plans to meet those needs.
  • Implement care plans and provide education to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Document all patient interactions, assessments, and care provided in a timely and accurate manner.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
  • At least six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
