Job SummaryWe are seeking a skilled Licensed Practical Nurse (LPN) to join our team at The Willows at Bellevue. As an LPN, you will play a vital role in providing high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure their physical, emotional, and social needs are met.
Key Responsibilities- Assessment and Care Planning: Contribute to the assessment of patients in close collaboration with Registered Nurses, identifying their needs and developing individualized care plans to meet those needs.
- Medication Administration: Administer medications and observe patients to ensure they do not have adverse reactions to those medications.
- Lab Tests and Therapeutic Services: Perform routine lab tests and therapeutic services as required.
- Patient Education: Educate patients and their families about recommended disease treatment plans, including self-care post-discharge, holistic health needs, available resources, and follow-up care.
- Safety and Emergency Response: Identify safety hazards and emergency situations to keep residents safe, taking corrective action as appropriate.
- Education and Licensure: Licensed Practical/Vocational Nurse obtained prior to hire date or job transfer date.
- Certifications: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date.
- High School Diploma or Equivalent: High school diploma or GED required.
