Nurse Practitioner

CVS Health Careers is seeking a highly skilled and compassionate Nurse Practitioner to join our Primary Care team. As a key member of our clinical care team, you will provide high-quality, patient-centered care to a panel of patients, working collaboratively with a dedicated team to drive better outcomes.
Key Responsibilities- Provide accurate and comprehensive assessments, diagnoses, and treatment plans for patients with various medical conditions.
- Conduct thorough patient histories and physical examinations, using knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions.
- Counsel and educate patients on health maintenance, disease prevention, and management of chronic conditions.
- Work effectively with the clinical care team to ensure seamless coordination of patient care, including referrals to specialists and patient care components.
- Document all patient interactions and care within an electronic health record, adhering to current policies and procedures.
- Master's degree in Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role.
- A minimum of 1 year experience as a Nurse Practitioner, with experience in Family medicine or primary care practice preferred.
- Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing.
- Current National Board Certification in Family Practice.
