Key Responsibilities- Provide evidenced-based care to patients, including accurate assessment, diagnosis, treatment, and management of health problems.
- Work collaboratively with a dedicated clinical care team to ensure seamless care coordination and high-quality patient outcomes.
- Counsel and educate patients on health maintenance, respond to patient care inquiries, and evaluate and treat patients' medical needs with safe and high-quality treatment.
- Document all patient care within an electronic health record in accordance with current policies and procedures.
- Participate in clinical care team huddles, demonstrate core values of CVS Health, and communicate effectively with members of the clinical care team, other health professionals, and health-related agencies.
- Experience in Family medicine or primary care practice preferred.
- Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
- Ability to work remotely in a clinical care team culture, work without direct supervision, and practice autonomously.
- Capacity to collaborate with professional colleagues frequently to ensure quality patient care.
