We are seeking a skilled Nurse Practitioner to join our team in a PRN/Part Time capacity. As a Home Risk Assessment Nurse Practitioner, you will conduct comprehensive health assessments, review medical history, and provide education to patients in their homes.
Key Responsibilities:
- Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
- Collaborate with physicians, case managers, and other healthcare professionals
- Provide education and follow-up care to patients
- Work in a variety of settings, including home visits, skilled facilities, and telehealth
Requirements:
- Master's degree in nursing or equivalent experience
- Current RN and NP licensure, with prescription authority
- Board certification in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
