Job Summary

The Community Care Specialist provides intensive support to youth and families in need of extra assistance through recreational, educational, and social activities.

This role is responsible for assisting the team with supervising, counseling, teaching, and supporting children, families, and foster families as needed.

Key Responsibilities

  • Provide individual treatment and supportive services to youth and families, including role modeling, homework assistance, and education activities.
  • Participate in the development of individual service plans.
  • Conduct family team meetings to discuss ongoing decision-making processes.
  • Act as an advocate for youth with community agencies and facilitate linkages to resources.
  • Maintain regular communication with guardians, foster parents, and biological parents of youth.
  • Collaborate with service providers to ensure best service delivery.
  • Document progress made on all goals, including educational, vocational, social/emotional, behavioral, and personal matters.

Requirements

  • Minimum High School diploma required; Associate's degree preferred.
  • Minimum 1 year experience working with youth and families, human services, social services, child welfare, or non-profit organizations.
  • Ability to work a flexible schedule, including evenings, weekends, and holidays.
  • Valid Driver's License with acceptable driving history and a reliable vehicle.
  • Travel required 60% of the time.
