Hospice Aide Job Description

At St. Croix Hospice, we are seeking a compassionate and skilled Hospice Aide to join our interdisciplinary care team. As a paraprofessional, you will work under RN supervision to provide quality, continuity, and safe patient care. Your responsibilities will include timely and accurate documentation of care, as well as providing patient care including bathing, nail/skin care, dressing/undressing, dental and other personal hygiene while maintaining a sanitary and orderly environment.

Key Responsibilities:
• Provide patient care, including bathing, nail/skin care, dressing/undressing, dental and other personal hygiene
• Maintain a sanitary and orderly environment
• Offer positive, respectful, and confidential patient/family communication
• Take and record vital signs, and assist with self-administered medications
• Plan and prepare nutritious meals, including shopping and feeding the patient as needed
• Assess and report patient's condition and changes to the RN Case Manager
• Assist in ambulation, exercise, transfers, positioning and support a complete range of motion and other therapy activities
