Role Overview

The Clinical Rehabilitation Specialist will play a vital role in delivering high-quality therapeutic care to patients. This position involves assessing patients' strengths and weaknesses to develop effective treatment plans. The successful candidate must maintain patient confidentiality while creating a positive and encouraging environment that promotes healing and recovery.

Responsibilities:

* Collects and records patient care data, adhering to guidelines of the patient care service, regulatory agencies, and third-party payers.
* Conducts initial evaluations, daily notes, reassessments/progress reports, discharge plans, and communicates with team members.
* Prepares for evaluations according to department policies/procedures and standards of practice, including obtaining patient history, researching unfamiliar diagnoses, and selecting/administering tests/measures.
* Synthesizes evaluation results to formulate diagnoses, goals, prognosis, and plans of care.
* Establishes treatment plans based on evaluation results, goals, and best practices, collaborating with interdisciplinary teams, patients, and support systems.
* Identifies needs for consultations/referrals to other healthcare professionals or services and initiates/discharge plans.

Requirements

* Bachelor's or Master's Degree required; Doctorate degree preferred.
* Licensed Physical Therapist in the state of practice.
* Completed state-regulated CEU requirements in the appropriate state (NY, NJ).
* Valid CPR License.
* Experience/knowledge with EMR software is preferred.
* Excellent oral/written communication skills.
* Strong clinical decision-making skills.
* Excellent work ethic and dedication to patient success.
* Basic computer skills.
* Works as a team member/individually with minimal supervision.
* Maintains a positive, professional demeanor at all times.
