Responsibilities
  • Provide skilled pediatric home care to patients in their homes, hospitals, or other healthcare settings.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Conduct thorough assessments, identify patient needs, and develop effective care plans.
  • Administer medications, perform treatments, and provide wound care as needed.
  • Document patient progress, maintain accurate records, and communicate with healthcare teams.
Requirements
  • RN/LVN with a current valid license to practice as issued by the State Board of Nursing.
  • Current CPR certification.
