The Clinical Pharmacist - Transition of Care is responsible for ensuring the safe, effective, and cost-efficient management of medication during the transition of care between the acute and ambulatory care settings.
Key Responsibilities- Collaborate with clinicians and other healthcare professionals to ensure seamless transitions of care.
- Develop and implement strategies to optimize medication use and reduce medication errors.
- Provide education and training to healthcare staff on medication management and transition of care best practices.
- Conduct regular audits to ensure compliance with regulatory requirements and CHOC's policies and procedures.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Minimum one (1) year of clinical pharmacy experience in an acute care setting.
- Preferred: Experience in a pediatric acute care setting.
- Bachelor of Science in Pharmacy degree or Pharm.D. from an accredited college or university.
- Basic Life Support (BLS) certification.
- Pharmacist licensure in the state of California.
- Completion of an ASHP-accredited hospital/clinical/pediatric residency program (PGY1 and/or PGY2).
- Knowledge of current trends in pharmacokinetics, pharmacology, and toxicology as they relate to drug therapy in the neonatal, pediatric, and adolescent patient population.
The Clinical Pharmacist - Transition of Care will work in a fast-paced, dynamic environment with a high level of energy and activity. The position requires the ability to lift and carry 40-50 pounds, push/pull up to 50 pounds, and stand for long periods of time.
