Job Summary

We are seeking a skilled RN/Flex Rehab Community Liaison to join our team at Mercy. This dynamic position is responsible for accepting referrals in various settings, including acute care, physician clinics, and community venues. The ideal candidate will possess excellent customer service skills, be able to work independently, and have a strong understanding of Hospice and Home Services.

Key Responsibilities
  • Accept referrals in acute care, physician clinics, and community venues
  • Maintain an active presence in these settings and deliver excellent customer service
  • Assist in facilitating timely discharges and support continuity of care within the Mercy Health System
  • Have a strong understanding of current guidelines, regulatory, and clinical in nature, and assist in determining client appropriateness for Hospice and Home Services
  • Communicate services to physicians and post-acute communities
Requirements
  • Registered Nurse required, Bachelor's degree preferred
  • Minimum three years of nursing experience required, with experience in Hospice and Home Health preferred
  • Experience in the in-patient acute setting and community setting preferred
  • Certification in Hospice and Palliative Nursing (CHPN) preferred
  • Basic Computer Skills and understanding of Electronic Medical Records required
