About the Role

We are seeking a skilled Occupational Therapist to join our Home Health team at AgeRight Care at Home. As an Occupational Therapist, you will play a vital role in delivering person-centered care to our patients, helping them achieve their therapy goals and improve their quality of life.

Key Responsibilities
  • Establish treatment plans in accordance with physician orders
  • Prepare and complete clinical and progress notes
  • Teach patients and their families/caregivers to meet individual treatment goals
Requirements
  • Licensed as an Occupational Therapist in the state of Oregon
  • Graduate from an accredited program with a degree in Occupational Therapy
  • CPR Certification
  • National Provider Identifier (NPI) required upon hire
Preferred Qualifications
  • Experience in Home Health
  • Organizational skills to facilitate program development
