We are seeking a highly skilled Registered Nurse Case Manager to join our team at CommonSpirit Health Mountain Region. As a RN Case Manager, you will play a critical role in ensuring the best possible care experience for our patients.
Key Responsibilities- Advocate for patients while collaborating closely with interdisciplinary healthcare partners, facilities, and families to ensure the best possible care experience from admission through discharge and beyond.
- Utilize clinical expertise, discretion, and independent judgment in assessing/reassessing, facilitating care coordination, utilization management, and patient advocacy.
- Assure medical appropriateness criteria are met for status and level of care.
- Nursing Diploma or Associates Degree in Nursing required; BSN preferred.
- RN License in Applicable State of Employment required.
- 1 year of nursing or case management experience required.
- Experience working with EMR preferred.
- Working knowledge of regulatory requirements and accreditation standards strongly preferred.
- BLS from the American Heart Association, required.
