Speech-Language Pathologist Job Description

We are seeking a skilled Speech-Language Pathologist to join our team at Santa Fe Care Center. This is a per-diem position, requiring flexibility to work as needed. If you have a passion for working with geriatric patients, we encourage you to apply.

Job Summary:

The Speech-Language Pathologist will provide speech therapy services to patients in their homes, as prescribed by the attending physician. This role requires a qualified professional to evaluate, plan, and provide rehabilitative services for speech and language disorders.

Key Responsibilities:
  • Evaluate patients' speech and language abilities, identifying defects and assets, and perform periodic re-evaluations.
  • Develop and implement individualized treatment plans to address speech and language disorders.
  • Record treatment progress and patient reactions in clinical/progress notes, which are written on the day of the visit and incorporated into the patient's record weekly.
  • Maintain accurate records of patient progress, including summary reports.
  • Collaborate with community resources and Agency personnel to ensure seamless care.
  • Select and administer diagnostic and therapeutic techniques and materials.
  • Provide education and counseling to healthcare team members and family members on methods to assist patients in improving, correcting, and accepting their disabilities.
  • Attend paramedical service meetings and combined paramedical and nursing service meetings.
  • Participate in case conferences and contribute to the development and revision of the physician's plan of treatment.
  • Communicate effectively with all care providers.
  • Confirm visit scheduling with the DCS/Case Manager on a weekly basis to coordinate necessary visits with other personnel.
  • Notify the Agency of absences due to illness, emergency leave, normal vacation periods, or special professional meetings that may impact agreed services.
Qualifications:
  • Membership in the American Speech and Hearing Association and certification by the Association.
  • Meet the educational and experience requirements for a Certificate of Clinical Competence (CCC) in speech pathology or audiology granted by the American Speech and Hearing Association.
  • At least a master's degree is required, with one full year of supervised work experience.
  • Licensure or certification by the state, if required.
  • New graduates are welcome; no experience is necessary. Must be able to complete point-of-service documentation and provide group and concurrent therapy as scheduled.
Functional Abilities:
  • Ability to read 12-point or larger type.
  • Ability to hear and speak in a manner understood by most people.
