Job Title: Home Health Social Worker

Job Responsibilities
  • Conduct comprehensive psychosocial assessments of patients and families to inform care plans
  • Provide individual and family counseling to address mental and emotional needs
  • Identify and address special needs related to cultural and religious diversity
  • Evaluate and address financial and environmental concerns
  • Provide crisis intervention support and liaison services as needed
  • Collaborate with healthcare teams to develop and implement individualized care plans
Requirements
  • Master's degree in social work from a Council on Social Work Education-accredited program
  • Active licensure as a Social Worker in the state of employment
  • Minimum one year of experience in healthcare, with hospice or homecare experience preferred
  • Reliable transportation and valid driver's license
