- Coordinate Services: Collaborate with healthcare professionals, family members, and caregivers to ensure seamless delivery of care services.
- Provide Direct Care: Administer medications, perform treatments, and provide emotional support to patients and their families.
- Communicate Effectively: Maintain open and transparent communication with patients, families, and healthcare teams to ensure informed decision-making.
- Monitor and Evaluate: Regularly review patient progress, adjust care plans as needed, and ensure timely communication with healthcare teams.
- Education and Licensure: Graduate of an accredited school of nursing with a current RN license in the state of practice.
- Experience: One to two years of recent acute care experience in an institutional setting, with a preference for home health care experience.
- Certifications: Current CPR certification and ability to obtain any additional certifications as required by the organization.
- Skills: Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
Prolonged or considerable walking or standing may be required. Ability to lift, position, or transfer patients, as well as lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching may be necessary. Visual acuity and hearing to perform required nursing skills.
