Advanced Practice Clinician for Primary Care Team

The Southeast Medical Group seeks an experienced Advanced Practice Clinician to join our primary care team. As a key member of our collaborative healthcare team, you will provide high-quality patient care, working closely with physicians and other healthcare professionals.
About the Role
This is a dynamic opportunity for an autonomous clinician who can work independently in our outpatient setting, managing a diverse patient population. You will diagnose and treat various conditions, prescribe medications, and counsel patients on disease management. Your expertise will be valuable in providing evidence-based care and contributing to our organization's strategic goals.
Responsibilities
- Diagnose and manage acute and chronic medical conditions, including diabetes, hypertension, kidney disease, and other complex health issues.
- Order diagnostic tests, interpret results, and prescribe treatments as necessary.
- Counsel patients on disease prevention, treatment, and expected outcomes.
- Work collaboratively with our multidisciplinary team to deliver comprehensive patient care.
- Participate in quality improvement initiatives and contribute to the development of best practices.
Requirements
- Master's degree from an accredited Physician Assistant or Nurse Practitioner program.
- Current licensure and prescriptive authority in Georgia.
- Experience working in primary care settings, preferably in a similar role.
