The Clinical Pharmacist Specialist (CPS) is a pivotal role within the healthcare framework, dedicated to enhancing patient outcomes through expert pharmaceutical care. This position operates at the forefront of clinical practice, engaging in both direct and indirect patient interactions to ensure optimal medication management.
Key Responsibilities:
- Engage actively in multidisciplinary team discussions to contribute to comprehensive patient care.
- Assess and validate medication orders from physicians for suitability and effectiveness.
- Collaborate with healthcare professionals to formulate and implement individualized drug therapy plans.
- Monitor patient responses to treatments, providing timely recommendations for adjustments to medication regimens as necessary.
- Authorize new medication orders, including the review of chemotherapy protocols and management of non-formulary requests.
- Participate in reimbursement optimization efforts to reduce insurance claim denials.
- Review medication use criteria and ensure compliance with medical necessity and appropriate indications based on payer guidelines.
- Stay informed about changes in payer policies and their implications for patient care.
- Provide education to medical, nursing, and pharmacy staff regarding the management and monitoring of oncology therapies.
- Collaborate with the medication safety officer to address and resolve medication errors and near misses.
- Contribute to formulary management and medication use processes to enhance patient care delivery.
- Ensure adherence to all regulatory standards and organizational policies governing pharmacy practice.
- Perform additional duties as directed by leadership.
Qualifications:
- Graduation from an ACPE-accredited School of Pharmacy.
- Completion of a Pharmacy Practice Residency (PGY1) and a Specialty Residency (PGY2), or equivalent experience in clinical pharmacy.
- Possession of a current CA Pharmacist License or eligibility within three months of hire.
- BLS certification is required.
- Board Certification in the relevant specialty is preferred or must be obtained within three years of employment.
- A minimum of five years of clinical pharmacy experience in the specified specialty is preferred.
