As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to elderly Veterans with complex geriatric medical conditions and dementia.
Job SummaryWe are seeking a highly skilled and compassionate Physician (Geriatrics-PACT) to join our team. The ideal candidate will have a strong background in geriatric medicine, with experience in dementia care, mental health illness management, rehabilitation, transitions of care, and healthcare systems navigation.
Responsibilities- Lead the Interdisciplinary Geriatrics team in the East Bay based at Martinez, 11CA to achieve the Service's mission and goals.
- Provide specialty interdisciplinary Geriatrics expertise to complex geriatric patients, frequently at high risk of frequent hospital re-admissions and Emergency Department visits.
- Collaborate with a core interdisciplinary team consisting of a Geriatrician (MD, DO), Nurse Practitioner, Registered Nurse, Pharmacist, and Social Worker integrated with the Geriatrics team.
- Report to the Medical Director of Geriatrics Section, under Geriatrics and Extended Care Service, and share on-call responsibilities with other Geriatricians in the section.
- U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Must be proficient in written and spoken English.
- Must pass pre-employment physical examination.
- Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- Participation in the Coronavirus Disease 2019 (COVID-19) vaccination program is a requirement for all Veterans Health Administration Health Care Personnel (HCP).
- Board certification in Geriatric Medicine.
- Possess geriatric medicine expert with knowledge and practice experience in geriatric medicine, dementia care, mental health illness management, rehabilitation, transitions of care, and healthcare systems navigation.
