Job Summary

We are seeking a skilled Nurse Care Specialist to join our team at Highland Health and Rehabilitation of Cascadia. As a key member of our healthcare staff, you will be responsible for providing high-quality patient care and ensuring the well-being of our residents.

Key Responsibilities
  • Provide Patient Care: Supervise staff, monitor patients, implement treatments, develop patient care plans, administer medications, and follow up with families to provide the highest quality of care.
  • Document Patient Care: Document care performed and observations of resident status in the clinical record promptly, utilizing standard clinical documentation guidelines as required by the facility, and local, state, and federal rules and regulations.
  • Communicate with Physicians: Communicate with resident's physician to ensure seamless care coordination.
  • Medication Management: Participate in medication management to promote optimal safety and effectiveness for residents by administering medications as ordered, preparing, administering, and documenting medications as prescribed, observing and reporting resident responses to medication, identifying and promptly communicating adverse drug reactions, and initiating enteral or parenteral therapy as ordered.
  • Emergency Response: Assist with or institute emergency measures for sudden adverse developments with residents, direct and administer cardiopulmonary resuscitation, and ensure punctuality and regular attendance for assigned shifts.
Requirements
  • Education: Graduate of an accredited RN School of Nursing. BSN preferred.
  • Licenses/Certification: Current RN Licensure in the State employed. Valid CPR Certification.
  • Experience: Six months experience in a long-term care environment preferred.
