Job Summary:


Provides comprehensive psychosocial assessment, care planning, and intervention services to patients and families in the emergency department setting.


Key Responsibilities:
Collaborates with the care team to develop and support patient care plans, identifying barriers and developing safe dispositions. Serves as a patient advocate, supporting shared decision-making and the use of decision aids. Educates patients and families on accessing medical home services, establishing self-management goals, and navigating the healthcare system. Coordinates and aligns patient care and community resources to facilitate patient care transitions and prevent hospital readmissions and emergency department visits. Appropriately documents consults and interventions in the patient's electronic medical record. Maintains compliance with organizational policies and procedures and evidence-based guidelines.

Requirements:
Master's Degree in Social Work. LCSW preferred, must obtain within 3 years of hire. Minimum of two years paid experience in social work. Experience in clinical/medical settings and crisis intervention.

Work Details:
Per Diem. Variable work schedule. 8.00 standard hours per week. Exempt from overtime: No.
