About the RoleThis is a unique opportunity to join our team of compassionate professionals who are passionate about making a difference in the lives of our patients. As a Clinical Assessment Advisor, you'll be responsible for:
- Conducting thorough assessments of patient needs, identifying areas for improvement, and developing personalized care plans
- Collaborating with our interdisciplinary teams, including nurses, therapists, and social workers, to ensure comprehensive care coordination
- Communicating effectively with patients, families, and healthcare providers to ensure seamless transitions and optimal care
- Monitoring patient progress, adjusting care plans as needed, and ensuring timely interventions to prevent complications
- Participating in quality improvement initiatives to enhance patient outcomes and satisfaction
To be successful in this role, you'll need:
- A Bachelor of Science degree in Nursing (BSN) or a related field
- At least 2 years of clinical experience in home health or a related setting
- Current CPR certification and a valid driver's license
- Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
- Knowledge of state and federal regulations governing home health care, as well as industry standards and best practices
