Responsibilities:- Participate as an integral member of an interdisciplinary treatment team led by a physician.
- Provide direct one-on-one age-specific psychotherapy services to patients as requested.
- Assist medical staff in determining appropriateness of services and receive referrals for intervention.
- Determine the developmental age of patients and assist the treatment team in developing and implementing treatment plans.
- Assess psychosocial needs, including high-risk indicators, and document obtainment of psychosocial history/assessment.
- Correlate all assembled data on the psychosocial history in a clinically pertinent manner.
- Perform continued-care planning with the treatment team from the first day of services.
- Provide group therapy as deemed appropriate.
- Act as a liaison for patients and their families to various community agencies, consultants, and services.
- Record and chart all patient contacts as required.
- Provide direct family therapy to patients and their families as directed by treatment planning and program requirements.
- Maintain appropriate certification, licensure, and competency through supervision and/or training and successful completion of annual performance evaluations.
- Attend continuing education seminars and in-services as necessary to maintain professional licensure, competency, and meet hospital required educational standards.
- Master's Degree in a related field.
- Licensure in a related field.
- West Virginia state criminal background check and federal, if applicable, required for DHHR regulated area.
- AVADE training required within 90 days or hire.
- Advanced certification preferred when applicable.
- Obtain certification in HeartSaver First Aid within 30 days of hire date.
The work environment includes minimal time in the office at Chestnut Ridge Center (no more than 30% of the work week). The majority of the time, the employee will be in the homes and living spaces of the community-based clientele of the program.
This will include areas that have sub-par hygienic characteristics.
