Job Summary:

The Medical Social Worker - Patient Care Specialist will assist the physician and other healthcare team members in understanding the significant social and emotional factors that may contribute to patients' health problems. This role will participate in the development of the plan of care and provide services necessary to assist in resolving social/emotional problems that impede effective treatment and recovery.

Key Responsibilities:

  • Assess social/emotional factors related to illness, need for care, response to treatment, adjustment to care, relationship of medical and nursing requirements to home situation, and financial resources and availability of community resources.
  • Obtain appropriate available resources, including community, financial, and caregiver resources.
  • Counseling services to promote beneficial treatment of individuals' medical conditions.

Requirements:

  • Masters Degree from a social work curriculum accredited by the Council on Social Work Education or a Masters Degree from a school of social work accredited by the Council on Social Work Education.
  • Current Oregon licensure as a clinical social worker (LCSW) or certification by the Academy of Certified Social Worker (ACSW) or be in process to acquire either LCSW or ACSW.
  • Current Drivers License, dependable transportation, and current CPR certification.
